We understand the individual(s) signing this application and the use group; will be held responsible for all equipment, the general cleanliness of the area after use, and any damages other than normal wear and attrition of fixtures and equipment. This responsibility also includes the turning off of all lights, except security lights.

We have been furnished a copy of the "House Rules", (scroll down below this application), agree to abide by them, and expressly understand and agree that a violation of them may be grounds for future refusal to rent.

Please note that as of 2005, Lions Park is proud to be SMOKE FREE ZIONZVILLE. The entire grounds of Lions Park and our rental facilities are SMOKE FREE.

Please return two copies to:

Zionsville Lions Club, c/o Rental Chairperson, P.O. Box 252, Zionsville, Indiana 46077.

The duplicate copy will be returned as confirmation of date requested.
 

Note: Enclose a deposit check for (Circle one:  $50 (Gazebo & Shelter House) or $150 (Club House).  In the event of cancellation, the fee will be returned upon notification at least two weeks prior to scheduled use. If the cancellation is not received within two weeks of scheduled event the fee may be retained by the Lions Club.
 

No deposit is required from Lions Club members but a donation is requested for the use of the Gazebo and Shelter House. Lions Club members may use the Club House for a minimum of $45.
